Requests for Student Information and Release of Information

Procedures regarding release of student information are for the protection of our students. Any request for the release of information pertaining to a student's record must be made in writing to the Student Services Office. Fax and e-mail (with PIN number) requests are accepted. This includes requests for transcripts, financial information, class schedules, receipts, grade reports, letters of verification, etc. Any request for release of information pertaining to a student’s record must include the following:

  • Student’s name
  • Student ID number
  • Address
  • Telephone number
  • Reason for release
  • Party to whom the information is to be released
  • Address and/or fax of party to whom the information is to be sent
  • Student’s signature.
A Release of Information Form may be used to request information or release of information relative to a student’s record.

Forms are available from the Student Services Office.
